- Board certification/eligibility in Family Medicine (MD or DO)
- Current licensure by the Massachusetts Board of Registration in Medicine
- ACLS or CPR/AED certified.
- Current DEA, DEA-X and Massachusetts Controlled Substance licensure for prescriptive authority
