    Job Description


    We, at Communities of Care, offer World Class Home Care with small community values.

    We're looking for a Full Time OR Part Time Registered Nurse (RN) or Licensed Practical Nurse (LPN) to join our team.

    We have several clients available around the state.

    New grads welcome
    We absolutely love our caregivers and are excited for you to join our winning team.


    Details for this case:

    • 14Monthold Female
    • Born Premature at 25 weeks
    • Located in Blaine, MN
    • Days and Overnights Shifts Available.
    • Trach/Vent
    Looking for one additional part time nurses to join a great team.


    Responsibilities:

    • Administer nursing care to client
    • Help maintain patients treatment plans
    • Monitor and report changes in patient symptoms or behavior
    • Maintain accurate patient medical records
    • Provide advice and support to patients and their family members

    Qualifications:

    • Work from a place of caring and honoring the dignity of all clients.
    • RN or LPN Minnesota License
    • Ability to build rapport with patients
    • Strong problem solving and critical thinking skills
