    Summary:

    The PRN Social Worker will work collaboratively as part of an innovative team to help to grow a new serious illness organization serving SC. The PRN Social Worker exemplifies the mission, vision, values, of Caring Seasons Health by:

    • Delivering excellent patient care experience.
    • Being fully present with patients/families and focusing on how we make them FEEL.
    • Performing psychosocial assessments on patients/families to identify support systems and barriers to care.
    • Managing distressing symptoms utilizing non-pharmacological interventions.
    • Providing patient/family counseling with regards to family dynamics, quality of life goals, end of life wishes, and grief.
    • Assessing coping skills of patient/family and providing education to promote healthy coping strategies.
    • Assisting in development of short-and long-term goal planning.
    • Effectively collaborating with interdisciplinary team to meet needs of patient/family.
    • Identifying and contributing to patient safety initiatives.
    • Ability to educate and assist with end-of-life planning/advance directives.
    • Make referrals to appropriate agencies on behalf of the patient and family.
    • Managing & communicating patient/family expectations.
    • Evaluating primary caregiver ability to provide care.
    • Conducting appropriate goals of care conversations with patients/family.
    • Demonstrating comfort level with difficult conversations.
    • Evaluating bereavement needs of family/caregiver and developing bereavement plan of care.
    • Delivering culturally competent care.
    • Completing accurate and timely documentation.
    • Providing spectacular customer service for patients, families, and referral sources.

    Knowledge, Skills & Abilities:

    • Knowledge of federal, state, and local regulations r/t Hospice.
    • Knowledge of Philosophy of comfort, dignity, autonomy, quality of life, and empowerment.
    • Knowledge of common grief responses.
    • Practices within the scope of license/certification.
    • Demonstrates ability to function in variety of care environments including home settings, skilled nursing centers, assisted living communities, etc.
    • Practices safety and infection control methods.
    • Proficient in Microsoft Office, Zoom / TEAMS.
    • Excellent problem-solving skills.
    • Strong organizational skills and ability to prioritize tasks.
    • Excellent interpersonal skills.
    • Ability to plan, prioritize and coordinate.
    • Ability to effectively work virtually when needed.

    Qualifications:

    • Master of Social Work degree.
    • LMSW.
    • Valid driver's license and proof of automobile insurance (or other mode of personal transportation).
    • Prior healthcare experience serving seriously ill patients – Preferred.
    • Prior home health experience a plus.
