    Description:
    Position Summary

    This is an onsite Tier 1 Extra Help Discharge Planning HURC II Position.

    Duties & Responsibilities

    Facilitates coordination of patient services in assigned patient care unit, or department.

    Collaborates with the patient, family, physicians, RNs, and multidisciplinary team to assure_x000d_ appropriate safe, cost effective, timely and efficient services.

    Utilizes clinical and social work experts_x000d_ as needed to ensure delivery of comprehensive patient services.

    Collaborates and communicates with all staff in developing and implementing the discharge planning_x000d_ process including:

    appropriate CLOC, continued stay, patient assessment (for appropriate Clinical_x000d_ Level of Care (CLOC), continued stay, planning, implementation, interdisciplinary collaboration and_x000d_ ongoing evaluation.

    Documents risk assessment and discharge planning information in the medical record according to_x000d_ department standards._x000d_ . Coordinates and provides discharge planning. Screens patients for high risk discharge planning needs_x000d_ and assures collaboration with interdisciplinary team members_x000d_


    Qualifications:
    Minimum Qualifications


    • Graduation from an accredited professional nursing program
    • Current licensure as a Registered Professional Nurse with the Illinois Department of Financial and Professional Regulation, Division of Professional Regulation.
    • Five years (60 months) of RN experience.
    • Current AHA BLS/CPR and other required unit specific certifications
    • Effective communication skills (oral and written) that facilitate therapeutic relationships between patients, family, and health care providers
    • Intermediate computer skills with the ability to become proficient in the Electronic Health Record (EHR) with on-the-job training
    • Excellent attendance, interpersonal skills and work history required.
    Preferred Qualifications

    Hospital Discharge Planning/Case Management Experience StronglyPreferred
