    Job Description:Are you passionate about providing support and caring for individuals with developmental disabilities? Are you looking for an organization where we help our clients achieve their goals and dreams? North Bay Regional Center is a nonprofit private corporation that contracts with the Department of Developmental Services to coordinate services and supports for individuals with developmental disabilities. We are looking for Social Services Case Managers to join our dedicated team of client program coordinators. At North Bay, we're promoting opportunities by supporting choices for people with developmental disabilities; in the role of Social Services Case Manager, you will be responsible for managing the delivery of services to regional center clients including those with intensive physical or emotional needs, ensuring that the client's Individual Program Plan and its objectives are pursued.

    Job Responsibilities: Our Social Services Case Managers are responsible for using person-centered planning to assist clients in the use of public and private resources before using regional center funds to purchase services. Additional responsibilities of the Social Services Case Manager include:
    • Monitoring vendor services and verifying that services are delivered in accordance with the purchase of service authorizations
    • Evaluating the effectiveness of clients' Individual Program Plans; ensuring that appropriate community resources are made available to the client
    • Conducting annual reassessments of clients' developmental status in conjunction with the interdisciplinary team
    • Conducting client assessments on an occasional basis
    • Job Requirements To qualify for the Social Services Case Manager role you must enjoy working with clients and assisting people with developmental disabilities.

    Additional requirements of the Social Services Case Manager include:
    • Bachelor's Degree in a field which offers content related to developmental disabilities (i.e. Social Work, Psychology, Counseling, Special Education, and/or Nursing)
    • Master's Degree preferred
    • 3 years of experience working with individuals with a developmental disability
    • Bilingual language ability (Spanish to English) is highly desirable
    This position will be primarily based in the office; however remote work may also be occasionally available.
