    *Job Title: *Home Health RN

    *Assignment Length:* 13 weeks with possible extension

    *Location:
    • Robinson, IL
    *Shift: *Days, 8 hour shifts with weekend on-call rotation every 3 weekends.

    *Pay Rate (Local): *$57/hour

    *Gross weekly pay rate:* $2,062/week with $1,162.00 non-taxed portion.

    *\*\*Accepts local or travel candidates \*\***Job Summary:

    The RN is responsible for assessing, planning, coordinating, implementing and documenting care for an assigned caseload of patients in the Home Health department.

    Develops and implements a home health plan that meets each patient's specific needs and is in compliance with regulating agencies, federal and state regulations, reimbursement guidelines and agency policies and procedures.

    *Duties and Responsibilities:
    • Performs accurate and complete assessment of patients; performs initial admission assessment; coordinates and ensures completion of interdisciplinary screenings and assessments; assesses and reassesses pain; assessments are age, culture and psychosocial appropriate; recommends appropriate referrals to other disciplines/community resources.
    • Formulates a plan of care based on patient needs, modifies the plan of care based on patient outcomes; formulates a teaching plan based on identified learning needs and evaluates the effectiveness of learning to include: educating the patient/family regarding treatment plan and providing discharge planning/instructions.
    • Makes comprehensive nursing decisions based on the interpretation of facts and evaluation of patient outcomes; participates in care conferences to discuss multidisciplinary team responsibilities, patient progress, plans for continued care, new problems; utilizes time management skills, able to complete care for full load of patients within scheduled work hours; utilizes pain management techniques; communicate with physician to obtain/renew/update orders to reflect changes in patient condition.
    • Prepares and administers medications based on age / clinical condition of the patient; starts intravenous lines for medications and fluids; maintains PICC / Central parenteral lines per protocol.
    • Perform other job duties as needed or assigned by the client.
    *Experience/Skills:
    • Acute Care Nursing minimum 1 year Preferred
    • Home Health Nursing minimum 1 year Preferred
    • Strong computer skills with the ability to navigate EMR programs
    • Excellent communication and customer service skills
    • Ability to work independently, prioritize and complete tasks in a timely manner
    • Knowledge of state, federal, accreditation and regulatory agency laws and requirements governing home healthcare
    *Job Requirements:
    • Active State RN License
    • Current BLS/CPR Certification (preferably from AHA)
    • Valid Driver's license and proof of personal auto insurance Required
    • Fully Vaccinated against Covid-19
