Description
RN Home Health
Roles & Responsibilities
Promote personal and patient safety.
Use effective customer service/interpersonal skills at all times.
Meet established productivity standards.
Use established program-specific criteria to determine eligibility for certification/recertification and placement in the appropriate level of care.
Provide outcome-based services for an assigned patient caseload from admission through discharge using a framework for assessment, implementation, coordination, and evaluation.
Perform initial and ongoing comprehensive assessments of client needs, including Outcome and Assessment Information Set (OASIS) assessment when required.
Initiate the plan of care, identifying problems, setting goals, and planning for discharge.
Revise goals and discharge planning as necessary through evaluation of outcomes.
Provide services and/or treatments requiring substantial and specialized nursing skills, including preventive, rehabilitative, palliative, and end-of-life nursing care.
Counsel clients and families in meeting nursing and related needs.
Initiate appropriate preventive and rehabilitative nursing procedures.
Coordinate and refer services including Home Health Aide, Occupational Therapy, Physical Therapy, Speech Therapy, Social Work, Chaplain, and Bereavement Care as appropriate.
Supervise LPNs and paraprofessionals providing services.
Participate in in-service programs.
Inform the physician and other team members of changes in the client's needs, condition, or service requests using Munson Home Health's current processes and technology.
Consult with and educate patients and families regarding the disease process, self-care techniques, disease/symptom management, palliative, and end-of-life care as appropriate.
Secure equipment and supplies required based on the plan of care, including pharmaceuticals for Hospice patients.
Evaluate own needs and seek personal support and/or guidance of management or another member of the interdisciplinary team as needed.
Record evaluation data, the plan of care, interventions, client's response to interventions, and all other clinical patient documentation in the electronic medical record in a timely manner.
Minimum Qualifications
Miles Traveled: miles/week
Caseload: 20-25 patients with 3-6 visits per day (Start of Care generally equivalent to 2.5 routine visits)
Certifications: Basic Life Support (BLS)
Experience: 1-3 years required
Work Authorization:Green Card, US Citizen
Weekend Requirement: 1 weekend out of a 4-weekend schedule
Primary Office Location: Traverse City, with primary work in Leelanau, Grand Traverse, and Benzie Counties
Starting Date: June 02, 2024
Duration: 3 months term
Hourly Rate: $65.52/hr
Schedule:10-hour shift, 4 days per week from 07:00 - 17:30
Travel Requirement: Only travelers staying above 75 miles from the facility are considered
