Inova Health System is looking for a Registered Nurse (RN) specialized in Case Management for a nursing position in Fairfax, Virginia.
Job Details & Requirements
The RN Case Manager 1 is responsible for providing discharge planning and ensuring continuity of care for assigned patients in both acute and post-acute settings. They serve as a vital link between patients, families, and various members of the healthcare team. Utilizing utilization management techniques, they assess the medical necessity, appropriateness, and efficiency of healthcare services, procedures, and facilities. The role involves timely regulatory compliance, overseeing precertification, and handling payer authorization processes when required. Additionally, active participation in clinical performance improvement initiatives is expected.
Key Responsibilities
- Collecting and analyzing data related to performance and outcome indicators, such as resource utilization and financial metrics. Addressing variances from care plans and collaborating with healthcare professionals to ensure optimal patient outcomes. Using this data to implement changes that enhance practice and drive positive results.
- Collaborating with different departments to guarantee effective patient management, adhering to budgets, policies, and quality standards. Supporting individual growth by fulfilling education requirements and contributing to departmental goals that align with organizational success.
- Working on discharge planning and continuity of care for patients in acute and post-acute settings, coordinating referrals to various healthcare services and facilities as necessary. Collaborating with the healthcare team, patients, and families to assess discharge needs and provide ongoing post-discharge support.
- Actively communicating with internal and external case managers, coordinating services, and serving as a liaison between patients, families, and healthcare team members. Advocating for patients, addressing barriers to care, and ensuring the efficient utilization of healthcare services.
- Assessing patients' clinical and psychosocial needs, facilitating communication among all involved parties, and seeking consultations whenever needed to expedite care. Ensuring that all aspects critical to patients' care plans are effectively communicated.
- Other duties as assigned.
Requirements:
- Bachelor of Science in Nursing (BSN). ADN candidates must complete BSN within two years of employment.
- Licensed or eligible for licensure as a Registered Nurse in Virginia.
- Minimum of one year of Case Management or clinical care experience.
- Basic Life Support (BLS) certification through the American Heart Association
