    Position Summary:

    Reporting to the Program Manager for Substance User Health Services, the Outreach Coordinator is a critical position to engage the community members with the SUD programming at the health center.

    The position will also have the support of a community of other similar staff at other health centers through the Mass League-MGB SUD grant.


    Primary Responsibilities:


    Conduct visits in the community to provide information on CHC substance use services and the importance of SUD education and awareness.

    Not limited to providing flyers, cards other awareness tools that will be accessible to community member Facilitate and/or deliver communications, presentations about CHC substance use services atcommunity spaces including open to community meetings, tabling, public forums.

    Develop and integrate a community resource document for patient availability. Advocate for the integration of this document in all areas in the health center.

    Provide ongoing support and follow up to community businesses and spaces already or newly connected with for future collaboration events and other SUD awareness and education opportunities Establish and maintain a SUD leadership team with community members that are interested in addressing SUD in their communities.

    Meet with them on a frequent bases dependent on group.

    Facilitate in the process of the community feedback forum selected by the grant implementation Team Initiate and sustain the community building and trusting relationship with the community organizations, spaces and the staff, and community membersMaintain a focus on health equity in all programmatic implementation and outreachGain and share information about specific health topics most relevant to the population being servedUtilize organization standards as the basis for decision makingPerform other duties ONLY within the duties of a community engagement staff person.


    QualificationsSkills and Qualifications:
    Experience working in a direct service organization, including working with clinical staff, SUD clients, and community organizationsOutgoing orientation and ability to engage with a wide range of community membersHighly motivated, organized individual who is able to manage workload independentlyBe able practice trauma informed care with patients with Substance Use DisorderEnjoys challenges and working with a diverse workforceExperience using an Electronic Medical RecordCommitment to substance user health and improving social determinants of health
