    Summary



    The gastroenterology nurse practitioner (NP) is responsible for managing patient care, writing orders, documenting patient care activities, and participating in patient care meetings. The gastroenterology nurse practitioner is responsible for, but not limited to, writing orders, ordering associated labs and creating individualized plans of care for patient management. The NP may be involved in assisting in consult management for coordination of care in the community by the Fayetteville VA .



    Duties


    Duties:

    The Advanced Practice Provider (APP) will be Nurse Practitioner (NP) within Gastroenterology. The APP would have primary areas of responsibility to involve the gastroenterology continuity and procedure clinics. This position will be fully qualified and certified to function in both a clinical and administrative role and in accordance with the guidelines and policies of the Fayetteville Department of Veterans Affairs Medical Center. This APP is delegated the authority and responsibility in his/her area of expertise to assume major clinical, consultative, educational, research, and administrative duties and assignments related to the care of Gastroenterology patients.

    1.The nurse is responsible and accountable for individual nursing practice and determines the appropriate delegation of tasks consistent with the nurse's obligation to provide optimal care (ANA Code of Ethics for Nurses).

    2. Independently plans and delivers technically competent care to patients with complex needs.

    a. Provides on-call telephone coverage for gastroenterology.

    b. Writes gastroenterology orders and orders related studies and medications. Orders and provides medical backup for test, if needed.

    c. Manages routine medical problems of patients. Identifies changes in patients' conditions and appropriately diagnoses, treats, consults, refers, and communicates in timely fashion with attending gastroenterologist and other health care providers.

    d. Safely and effectively uses equipment required for patient care. Reports equipment in need of repair.

    e. Recognizes subtle abnormal signs and symptoms and/or changes in patients' condition and responds appropriately.

    3. Makes assignments and directs activities of unit staff, (RNs, LPNs, students, PSAs, other team members).

    4. Formulates and implements a plan of care based on the ANA Standards of Nursing Practice.

    a. Conducts follow-up assessments within 3 months after the initial assessment. Conducts annual re-assessments for stable patients and at least monthly re-assessments for unstable patients. Conducts the initial assessment of new patients in collaboration with the staff gastroenterology as requested.

    b. Contributes to the development and implementation of a patient plan of care that addresses gastroenterology. Provides continuity of care for patients and manages the long-term treatment plan.

    c. Provides regular patient education concerning gastroenterology and associated conditions.

    d. Performs regular advanced care planning with patients using shared decision making including but not limited to discussion of code status and advance directives.

    e. Manages patient immunizations in accordance with local policy.

    f. Serves as a member of the patient's interdisciplinary care team. Participates in patient care meetings, semi-annual long-term patient care meetings, and other special meetings as requested.

    g. Includes the patient and family/significant other in developing the initial plan of care, establishing goals, and discharge planning. Updates plan of care as needed based on patient responses.

    h. Assesses patient's learning needs, learning preference, and readiness to learn.

    Clinical:

    • Primary focus of practice is the clinical management for patients in the Gastroenterology clinic. Specific clinical duties include:
      • Attends and manages general Gastroenterology Clinics
      • Completes Gastroenterology electronic consults.
      • Review of Gastroenterology consults for outpatient consultation and procedure clinics
    • Collaborates with other health care resources (Primary Care, General Surgery, Oncology) to ensure continuity of care.
    • Keeps abreast of latest trends, skills, procedures, and protocols relevant in the area of specialty and serves as a resource person for the medical center staff.
    • Utilizes and promotes approved standards of care, procedures, methods and techniques, safety precautions and protocols in their application to the gastroenterology patient population in all settings.
    • Collaborates with other health care resources to ensure continuity of care.
    Administrative:
    • Primary resource for outpatient Gastroenterology consults in collaboration with different services/personnel within the health care system:
      • Pre-anesthesia department - determining case appropriateness for the Endoscopy Suite vs Operating Room utilization.
      • Referral coordination team (RCT) RNs - determining eligibility for care in the community (CITC)
      • GI Endoscopy Suite RNs - placing appropriate consults as needed for upcoming Endo Suite procedures (admission consults, lodging consults, stop anticoagulation consults, pacemaker/AICD consults)
