What is PACE?
PACE, or Program of All-Inclusive Care for the Elderly, is an alternative care model for those over 55 years of age requiring nursing home level of care which aims to keep participants living at home in the community.
PACE is a collaborative, interdisciplinary approach which coordinates all levels of medical and social needs, including prevention, wellness, socialization, primary care, and access to specialists and other services.
The Speech-Language Pathologist (SLP) will deliver therapeutic services to participants according to the Illinois Speech-Language Pathology Practice Act. The SLP will report to the PACE Site Director.Primary Duties and Responsibilities:
Conduct and document a thorough assessment of the participant to determine and diagnose communicative deficits and equipment needs per the Illinois Speech-Language Pathology Practice Act.
Performs all functions and duties as required as a member of the PACE Interdisciplinary Team (IDT), including, but not limited to, participation in IDT meetings, family conferences, grievances, service determination requests, CMS audits.
Coordinates with IDT in developing individualized plans of care for each PACE participant.Develop and implement an individualized treatment program based on the initial evaluation and containing achievable functional goals.
Comply with evaluation, treatment, and documentation guidelines, and consistently maintain timely and accurate departmental records.
Instruct aides in appropriate procedures and provide supervision in accordance with applicable State law.
Revise the participant's treatment program, as needed, based upon participant's performance.
Document the course of treatment with progress notes and a concise discharge summary utilizing a prescribed format.
Orient participants to program services and treatment procedures appropriate to their needs. Inform participants of any potential risk during any procedure.
Work with nursing staff, participants, and caregivers to maximize participant's use of functional abilities.
Instruct participants and family/other caregivers about therapeutic procedures to be continued by the participant at home.
Attend and participate in other department/facility meetings, as required.
Participate in the PACE Quality Assurance and Process Improvement program, as indicated.
May be involved in training clinical and non-clinical staff with regards to diet consistencies and swallow precautions.
Work alongside the PACE Dietitian and PACE food service provider to provide input and make recommendations on individualized meal preparation.
Requirements:Must possess and provide annually valid driver's license and documentation of current automobile insurance at PACE organization-specified levels.
Bilingual (English and Spanish) required.
Master's Degree in Speech-Language Pathology.
Adherence to licensure requirements for continuing education and professional development.
Minimum one year working with older adult population.
Valid Speech-Language Pathology licensure in the state of Illinois.
Hold a current Certificate of Clinical Competence in Speech-Language Pathology (CCC-SLP) by the American Speech-Language-Hearing Association.
BLS from American Heart Association.
