Licensed Vocational Nurse

Department: Nursing
Reports to: Director of Nurses
FLSA: Non- exempt
POSITION SUMMARY:
Working under the supervision of a registered nurse, the role involves implementing the established care plan for a group of residents. Tasks include providing total care for chronically ill and technologically dependent residents, administering medications, carrying out treatments, offering resident/family education, and maintaining care records. Additionally, the position involves assisting in planning, coordinating, and delivering individualized resident care in line with facility policies.
POSITION RELATIONSHIP
Collaborating with nursing personnel at all levels, facility consultants, physicians, department managers, and other staff, to provide resident assessment education.
QUALIFICATIONS/REQUIREMENTS:
1. Licensure/Certification:
a. Must hold a current, valid Licensed Vocational Nurse license in California
b. Current Basic Cardiac Life Support (BCLS) certification
2. Education: a. Graduate of an accredited School of Nursing as an LVN
3. Experience:
a. Minimum of six months LVN experience in the last two years in a general acute care hospital, or equivalent competency related to the resident population of the facility.
4. Language Skills:
a. Proficiency in reading, analyzing, and interpreting technical information, along with clear verbal communication in English.
5. Mathematical Skills:
a. Ability to perform basic arithmetic operations and unit conversions with precision.
b. Capable of performing these operations using different units of weight and volume.
6. POSITION RESPONSIBILITIES/DUTIES: a. Providing resident care that addresses physical, psycho-social, and spiritual needs while considering cultural diversity. b. Conducting thorough assessments, differentiating normal from abnormal findings, and intervening appropriately as per standards. c. Collaboration with residents/families and interdisciplinary teams to create personalized care plans. d. Exhibiting sound clinical judgment in implementing and evaluating nursing aspects of care plans. e. Monitoring and reporting resident condition changes accurately and promptly to the relevant parties. f. Ensuring delivery of necessary care services tailored to each resident's needs. g. Proficiently administering medications, including pain management. h. Proficiently providing IV care if certified, following facility protocols. i. Skill in treatment administration as directed by physicians, including wound care and catheter management. j. Competently managing emergency situations following established procedures. k. Maintaining knowledge and executing activities to support resident health. l. Respecting resident rights by ensuring privacy and dignity in care provision. m. Identifying and addressing resident/family educational needs while monitoring effectiveness of education provided. n. Demonstrating age-specific care knowledge, including safety measures and effective communication. o. Understanding and monitoring signs of abuse/neglect, reporting and documenting incidents as required. p. Exhibiting clear and effective oral and written communication with various stakeholders. q. Ensuring timely and accurate documentation to meet regulatory requirements and effectively address inquiries or complaints.
