    The Home Health Occupational Therapist (OT) is accountable for assessing and evaluating patient care needs concerning functional status, daily activities, fine motor skills, home evaluations, adaptive equipment, and other occupational therapy requirements based on medical conditions.

    • Delivers services within the scope of practice defined by state laws governing Occupational Therapy, following the care plan, employing evidence-based methodologies, and working collaboratively with other healthcare professionals.
    • Assesses the patient's functional status, overall body system condition necessary for CMS documentation, and occupational therapy necessities. Collaborates with the physician to craft the therapy care plan.
    • Monitors, documents, and updates the nurse supervisor and physician on the patient's progress and any changes in their condition.
    • Performs patient assessments to gauge the level of functionality using diagnostic tests to determine functional abilities and prognosis.

    Licensing Requirements

    • Active Occupational Therapy licensure in the state of practice
    • Valid CPR certification
    • Current driver's license, vehicle insurance, and access to a reliable vehicle, or public transport
