    Job Description:

    To provide housing case management services and support to adult individuals who are homeless, or at risk of being homeless, with mental health, chronic health, substance use & co-occurring issues with the goal of obtaining and maintaining safe, affordable housing and residential stability in the community.

    • Promote agency core values in day-to-day activities
    • Work with individuals to locate safe and affordable housing in an area of their choice
    • Promote individual movement through program phases, including assisting customers to develop goals and target activities leading to sustainable housing and successful graduation
    • Purchase individual household furnishings for participant residences
    • Assist with obtaining benefits if needed, collect income, determine rental stipend per month
    • Maintain open communication with treatment and service providers, families, landlords, and other supports or networks
    • Assist in outreach and location of referred individuals
    • Responsible for recordkeeping, including progress notes, and other required paperwork
    • Must have own transportation and see individuals in person twice each month, preferably in their homes
    • Attend team and community-based team meetings

    Requirements:

    • Proficient in computer software such as MSWord, Excel, Power Point, Desktop Publishing, Outlook
    • Knowledge of Community Health Systems

    Education and Experience:

    • Associate Degree with 1 year's qualifying experience working with populations: mental health, substance use, chronic illness and/or co-occurring disorders
    • High School Diploma with 2 years' qualifying experience working with populations: mental health, substance use, chronic illness and/or co-occurring disorders

    Compensation:

    $17.46 - $22.26/hour
