    Description
    Oversee the delivery of high-quality ambulatory clinic care, ensuring efficient clinic operations for elderly patients.
    Direct day-to-day operations within the clinic under the guidance of the Clinical Services Director (CSD).
    Coordinate clinic workflows to ensure effective clinical services.
    Develop and manage clinic staff, providing coaching, supervision, and fostering engagement.
    Analyze data to identify trends and opportunities for improvement, making recommendations to enhance clinic operations.
    Ensure accurate documentation of interventions in participants care plans and adherence to workflows and policies.
    Evaluate the quality and effectiveness of coordination and services in collaboration with medical practice leadership.
    Serve as the audit lead for clinical and nursing elements during CMS/DHCS audit reviews.
    Operate a high-performing, safe clinic with efficient workflows and protocols.
    Hire, retain, coach, develop, and supervise clinic team members.
    Analyze data to identify trends and opportunities for improvement.
    Ensure accurate documentation of interventions and adherence to care plans.
    Evaluate the quality and effectiveness of coordination and services.
    Serve as the audit lead for clinical and nursing elements.
    Associate or Bachelor s degree in nursing.
    Minimum of two (2) years of experience overseeing an ambulatory clinic operation.
    Medical insurance coverage (Medical, Dental, Vision).
