    Physical Therapist (PT) - Home Health

    Middlesex, Suffolk and Essex Counties

    We are seeking a dedicated Physical Therapist to join our Home Health team. As a Home Health PT, you will work one-on-one with patients in their homes, providing personalized therapy to help them regain strength, manage pain, and achieve their goals.

    In this role, you will have the opportunity to make a significant impact on your patients' lives, working as part of a collaborative home health team to develop and implement individualized plans of care.

    The ideal candidate will possess excellent clinical judgment, strong interpersonal skills, and the ability to work autonomously in a fast-paced environment.

    About the Position

    • $55-60 per hour plus mileage reimbursement
    • 1:1 therapist-to-patient ratios for maximum impact

    Key Responsibilities

    • Provide home-based physical therapy to patients with mobility, strength, and functionality challenges
    • Collaborate with the home health team to develop and implement individualized plans of care
    • Observe and assess patient movements, conduct progress evaluations, and modify care plans as needed
    • Assist patients with exercises and hands-on therapy to manage pain and increase mobility
    • Educate patients and their families on therapy goals, exercises, and self-care

    Requirements

    • Graduate of an accredited Physical Therapist program and active PT license in Massachusetts
    • Minimum 1 year of physical therapy experience, preferably in home healthcare
    • CPR certification
    • Knowledge of state and federal home health regulations
