Hospice Social Worker and Case Manager at St. Luke's Health System in Meridian, Idaho, United States Job Description Overview At St.

Luke's, our dedicated team of Behavioral Health professionals strive to build a positive, supportive, and inclusive culture that delivers exceptional patient experiences.

What you can expect from this role + 4, 8 hour shifts per week + Fully benefitted with hospital contributions + Case Management and bereavement support + Will follow patients that are in inpatient hospice Qualifications + Education:
Masters degree

+ Licenses/Certifications:

Licensed Master Social Worker (LMSW) or Licensed Clinical Social Worker (LCSW) in Idaho Preferred medical social work experience Why St.
