    POSITIONSUMMARY:
    In collaboration with thepatient/family, physicians and interdisciplinary team, the CaseManagement Coordinator ensures patient progression through thecontinuum of care in an efficient and cost effective manner. TheCase Management Coordinator is responsible for identifying,initiating and managing optimal patient flow/throughput to enhancecontinuity of care, smooth and safe transitions, patientsatisfaction, patient safety and length of stay management. Supportand expertise are provided through comprehensive assessment,planning, implementation and overall evaluation of individualpatient needs. Care coordination, and discharge planning areaccountabilities of this role. Payer-based requests regardingindividual members are fulfilled, where appropriate andpertinent. The Case Manager adheres to departmental andorganizational goals, objectives, standards of performance andpolicies and procedures, continually assuring quality patient careand regulatorycompliance.

    Requirements

    CERTIFICATION/LICENSES:
    Licensed as a Registered Nurseto practice nursing in the state of New Mexico. Meets contacthour requirements for licensure, including all state requiredcourses.
    OR Currentlicensure as an LMSW/LISW/LCSW in New Mexico. Case Managementcertification highly desirable – ACM (Accredited Case Manager), CCM(Certified Case Manager, RN-BC (Board Certified in CaseManagement)
