    Family Outreach Services Program Supervisor

    Families and Youth Innovation Plus is hiring a full-time Family Outreach Services Program Supervisor

    The Family Outreach Services Program Supervisor has direct supervision and oversight of the Family Outreach Services Program (A CYFD contracted Child Welfare Prevention Program) in the Family Innovative Services Umbrella (Juntos). The supervisor is responsible for the day to day operations of the service delivery outlined in the contracted scope of work. In addition, they are also responsible for the day to day supervision of personnel assigned to the COS program. They supervise this program in accordance with agency policies and procedures and Medicaid, State, and other applicable Standards.

    Key Responsibilities

    • Oversee direct client services to individuals, children, and or families according to Families & Youth, Inc., (FYI), Children, Youth, and Families Department (CYFD) of New Mexico, Medicaid, and State of New Mexico policies and procedures, and other applicable standards
    • Oversee the outcomes and effectiveness of all programs aligned in CYFD contract
    • Ensure that database information,assessments, treatment plans, progress notes, weekly notes, data logs and web based surveys, and /or other agency forms are properly documented, reviewed and completed
    • Oversee all individual, child and family case management interventions included in the CYFD contract standards and scope of work and the tiered case management response
    • Oversee the number of CYFD referrals and expected work hours; recognizing travel time and time allotted for completing required documentation
    • Assist in the development of position descriptions; follow procedures to fill vacancies, including conducting interviews of applicants, complete employee performance evaluations and make recommendations for promotions/salary increases; document any employee performance concerns or disciplinary actions, as described in the personnel policies
    • Review and recommend for approval ;travel reimbursements, timesheets, leave requests, continuing education request, requisitions for purchases, etc. prior to submitting to Clinical Director and CEO for authorization and approval
    • Maintain and required licensure and professional development at highest level attainable for degree and ensure completion of continuing education requirements for level of licensure in a timely manner
    • Participate in and develop networking meetings, public presentations and media announcements, interagency trainings and staffing with agencies to include schools, CYFD Juvenile Justice Division and Protective Services Division
    • Complete monthly, quarterly and annual reports as required by contracts or FYI
    • Assist in research, writing and defending proposals for program funding and expansion of services
    • Participate in agency-wide after hours crisis on-call services unless otherwise exempted

    Qualifications and Experience

    • Bachelor's degree in Social Work from an accredited college/university and five (1) year of any combination of experience including working with communities on health or social service related matters, social work/case management experience, behavioral health and/or health care, OR a bachelor's degree in any other field from an accredited college/university and at least 2 years of any combination of experience, including working with the communities on health or social service related matters, social/case management experience, behavioral health and or health care. IF a potential candidate for employment does not meet the minimum requirements, a written letter requesting a waiver of the minimum requirements must be sent to CYFD Prevention Initiative Bureau Program Specialist.
    • Excellent communication skills, both written and verbal
    • Meets specified deadlines and manages time effectively
    • Provide services in the most culturally sensitive, appropriate and ethical manner
    • 1 year with a Bachelor of Social Work OR 2 years of any combination of experience, including working with the communities on health or social service related matters, social/case management experience, behavioral health and or health care. IF a potential candidate for employment does not meet the minimum requirements, a written letter requesting a waiver of the minimum requirements must be sent to CYFD Prevention Initiative Bureau Program Specialist.
