- Conduct Adult/Geriatric Assessments, medical history, diagnosis, treatment, health education, physician referrals, case management referrals, and clear documentation
- Collaborate with healthcare professionals, family members, and caregivers
- Work with a network of providers in various venues such as Home Visits, Skilled Facility Visits, and Telehealth Visits
- Master's Degree or equivalent experience with NP licensure
- Current RN and NP licensure with prescription authority
- Board certified in a Matrix approved specialty
- Minimum 1 year of Nurse Practitioner experience
- Strong computer skills and familiarity with medical record software
- Excellent verbal and written communication skills
- Flexible and adaptable to change
- Valid state driver's license and reliable vehicle
- Ability to travel as needed

Nurse Practitioner - Home Risk Assessments
Type: PRN-Flexible-Nurse Practitioner
Location: Home, SNF, Telehealth, and other community environments in Moorhead, Mississippi
Hours: Days, Evening, Nights, Weekend, Flexible Scheduling
PAY: $90.00 per completed Assessment, 25 hours or more/month-Flexible Scheduling
Benefits: PRN providers receive limited Voluntary Benefits
Mileage Reimbursement provided for all miles when completing 20 assessments/month, No show payment for 10 or more booked assessments/month
