Transitional Care Case Manager to:
Bridge inpatient to ambulatory care for medical, surgical, and trauma populations.
Coordinate with interdisciplinary teams to ensure remarkable healthcare.
Use clinical expertise to empower patients during post-acute hospitalization recovery.

Minimum - Bachelor's degree in Nursing. Preferred Masters degree in Nursing or Health Care related field.
Work Experience :
Minimum - Three (3) years relevant clinical nursing experience. Equivalent combination of education and experience will be considered. Preferred Surgical, trauma, and intensive care experience.
Licenses and Certifications :
Minimum - RN licensed in the state of Wisconsin. CPR certification. Preferred - ACMA certification as a case manager.
