    St.

    Luke's Health System in Boise is seeking a Full Time Nurse Navigator to join our SL Boise Heart Clinic team.

    At St.

    Luke's Clinic - Idaho Cardiology Associates, the team has over 150 years of combined experience in the treatment of heart disease and offers you comprehensive cardiovascular care.

    Clinic services include electrophysiology and arrhythmia exams and consultations, transthoracic and transesophageal echocardiograms, stress tests, and nuclear cardiology.


    This position will support the Atrial Fibrillation (Afib) program within Boise Heart Clinic assisting scheduling procedures and navigating care for this patient population.

    The support position will also assist in quality metrics such as the national registry and data collection.

    (4) nine hours shifts, Monday - Friday.

    Responsibilities


    The Nurse Navigator offers medical guidance, acts as a support group, and guides patients and their families through the treatment process.


    • Serves as a single point of contact for patients undergoing treatment,.
    • Serves as a liaison between the patient and their family and the patient's network of care providers.
    • Educates patient on diagnosis, treatment options, and prognosis.
    • Assists the patient in scheduling appointments, completing paperwork, or securing referrals.
    • Facilitates short term and/or long term interventions.
    • Possesses comprehensive knowledge of subject matter.
    • Handles complex issues and problems, and refers only the most complex issues to higher-level staff.
    • Other duties and responsibilities as assigned.
    Qualifications


    • Education: Bachelors of Nursing Degree
    • Experience: 2 years's of RN experience
    • Licenses/Certifications: Current RN License in the State of Practice
