    POSITION SUMMARY:

    Utilizes the nursing process to provide skilled, client care services in the home and coordinates overall client care in accordance with Agency policy and physician's orders. Participates in continuous quality improvement activities and educational experiences in support of Agency philosophy and objectives, as well as, Health System initiatives.

    EDUCATION/TRAINING:

    Graduate from an approved School of Nursing program. At least one year of experience as a professional nurse within the past three years in an acute care setting. Experience in home health, IV therapy, ER or oncology preferred.

    LICENSURE/CERTIFICATION:

    Current RN license from the State of Illinois. Valid State driver's license and proof of liability insurance required. Bi-Annual CPR certification.

    EXPERIENCE/SKILLS/ABILITIES:

    Successful completion of annual clinical, age and job specific competencies and skill verification tools required. Agility to effectively communicate orally and in writing in a caring, considerate and concerned manner. Ability to direct staff and organize work assignments. Leadership skills essential.

    ESSENTIAL FUNCTIONS:
    1. Provides direct and indirect nursing care utilizing the nursing process to clients in the home setting in accordance with Agency policies and procedures. Administers medications and treatments as prescribed by physician.
    2. Demonstrates critical thinking skills and exercises nursing judgment in evaluating and assessing client care situations in accordance with Agency procedures and physician's orders.
    3. Serves as coordinator of the health care team for client and communicates and collaborates with other health professionals and community agencies to assure the integration of services. Organizes, leads and participates in patient care conferences, meetings and case reviews as necessary.
    4. Appropriately documents medical record and maintains clinical notes and summaries per policy.
    5. Coordinates family involvement in the assessment, treatment, teaching and continuing care of the client and assists family in coping with traumatic illnesses ensuring the provision of an environment that meets the patient's daily living needs. Provides care and education incorporating age-specific, cultural, spiritual and psychological needs of clients and their families into the plan of care.
    6. Assist in cost-efficient inventory control of would and ostomy supplies for UW Health Northern Illinois Health System.
    7. Assists in the supervision, training, education, orientation and validation of competency of new employees, students, other healthcare professionals and community workers.
    8. Performs all aspects of care in an environment that optimizes patient and environmental safety, promotes infection control procedures and in accordance with applicable laws, regulations and compliance efforts.
