- Conducts community assessments to identify physical, mental, social and/or environmental deviations from the normal, responses and attainments of outcomes, using nursing skills of assessment, nursing diagnosis, planning intervention and evaluation.
- Assesses patient needs and then develops plans of care. Consults with health care team members, including participating in case management with health care providers.
- Counsels patients and/or families in the home, community, school and clinic regarding acute and chronic conditions and the application of health promotion and disease prevention practices.
- Participates in Performance Improvement activities, facilitates and promotes Government Performance Results Act activities, and is involved in facility accreditation activities.
