Job summary
We are seeking a compassionate and experienced Registered Nurse (RN) to join our Advanced Illness Management (AIM) Palliative Care Team . This is a field-based position that provides in-person and telephonic care coordination, education, and assessments to patients with serious or life-limiting illnesses.Responsibilities
- Conduct in-home and telephonic nursing assessments and screenings for palliative care patients
- Complete risk stratification assessments and determine frequency of visits based on patient needs
