- Demonstrate knowledge of childhood psychiatric disorders and trauma.
- Responsible for the overall provision of services including but not limited to support, information, skill building, advocacy, service linkage, referrals and crisis intervention.
- Conduct initial and ongoing comprehensive assessments.
- Prepare and revise care plans to reflect client needs and personal goals with a focus on maintaining health and wellness.
- Maintain regular contact with clients and collateral contacts providing face-to-face visits.
- Advocate for and support clients to ensure access to resources necessary to support wellness/self-management and decrease frequency of emergency room visits and inpatient hospital admissions.
- Monitor and coordinate all care for clients, including access and maintenance of medical insurance, linkage to treatment providers and community resources.
- Collaborate with community providers as part of a multi-disciplinary team to ensure goal-directed care planning.
- Involve child and family in the development and on-going review of individualized care plans.
- Conduct crisis prevention when needed and follow up accordingly.
- Maintain detailed, timely, and accurate record keeping and documentation of services.
- Work as part of a care management team, attend and participate in regular team meetings to provide feedback and share resource information relating to client needs, issues, and concerns.
- Participate in regular individual supervision to address concerns/issues and improve skill development.
- Attend periodic trainings to enhance skill level and learn about wellness self-management and best practice skills.
- Be responsible for reporting/coordinating daily office and field schedules with supervisor.
- Engage in discharge planning with youth and family.

Job DescriptionSchedule: Mon-Fri 9am-5pm with flexibility
Position Summary: Mental Health Workers in this program provide service access, advocacy, and supports for ages 5-21 years who have or are at risk of developing a serious emotional disturbance (SED) and their families residing in Suffolk County to enable them to live successfully in the community. Services provided include but are not limited to: support, information, skill building, advocacy, service linkage, referral and crisis intervention.
Essential Accountabilities:
OPERATIONAL
17.Provide a Flexible schedule required to meet the needs of the youth and families. Two evenings required a week.
OTHER RESPONSIBILITIES
18. Participation in program and division trainings.
19. Attend workshops and conferences as requested. OMH mandatory trainings required.
20. Responsible for maintaining (or having access to) a personal vehicle throughout the course of the work day.
21. Utilize his or her own personal vehicle to transport clients and other duties to meet the program needs.
22. Submission of car insurance policy before the expiration date.
23. Other duties as assigned by Administration.
Qualifications
Bachelors of Arts or Science Degree with two years of relevant experience (e.g. care coordination, mental health, youth and families, or related populations), or a Master's Degree with one year of experience. Valid Driver's License required
