Job summary
The job is to assist with the delivery of community support services for individuals receiving services through our Clustered Apartments Program. The worker will coordinate the delivery of mental health services and develop linkages with the community.Responsibilities
- Provide training to clients in daily living skills.
- Facilitate development and/or enhancement of personal support systems within the client's community.
- Assist clients in creating and participating in recreational activities as indicated or available.
