    Job Title: Housing Coordinator

    This is a non-exempt management position within the individual site. The Housing Coordinator is responsible for the day-to-day operations of one of the residential programs within Nation's Finest, including, but not limited to, case management and contract compliance. This position reports directly to the Site Director or Designee. Housing Coordinators working in the Behavioral Health Center will work in partnership with the Clinical Coordinator.

    Job Duties:
    1. Direct Supervisor to Residential Case Manager(s) and Residential Support Staff within the specific residential program and take on all duties related to day to day supervision (site specific).
    2. Establish a comprehensive intake and orientation process to determine program eligibility, assess unmet needs, and integrate clients into the program. Receives initial referrals and ensures all in-processing client documentation and waivers are completed and signed immediately upon intake to the program. Refers to Case Manager for completion of the intake process.
    3. In coordination with case managers and/or Clinical Coordinator, ensures Individual Treatment Plans (ITP or ISP) are completed within the timeframe of grant requirements and on-going quality assurance of electronic databases (Nation's Finest and outside entities when applicable). Responsible for program billing including oversight of the client daily sign-in sheets, completion of the VA entry and exit forms.
    4. Assist clients in completing their monthly program fee calculation worksheet (if applicable). Verify income and expenditure data and collect fees in the form of money orders. Verify and ensure that the client accumulates and maintains the mandatory savings balance that will be needed for permanent housing move-in expenses.
    5. In conjunction with the case management team, monitor residents for nutritional, safety and rule compliance issues. Coordinates with the Program Support Specialist on purchases of non-food, and incidental program items from other sources.
    6. Monitor condition of the facilities and furnishings in the residential program to ensure health and safety of clients, and submit needed repair/replacement requests to designee for further action. Make service appointments and monitor when contractors are performing approved repairs. Ensure that all emergency drills and life and safety inspections are conducted by staff according to the agency drill and inspection schedule.
    7. Oversee client UA schedule and ensure that UA specimens are picked up in a timely manner and that the program maintains an adequate amount of testing supplies at all times. In coordination with case management team, maintain documentation of all client medications, closely monitor client's prescribed usage, and drug/alcohol test results.
    8. Schedule and conduct (or designate) regular house meetings with the clients when applicable. Provide a written summary of such meetings to be discussed during weekly clinical staff meeting.
    9. Coordinate and consult with the case management team to establish a life skills program that enhances the quality of the program and the individual clients' success and well-being. Ensure that the case management team conducts a life skills program on a weekly basis (Supportive Housing Program Only).
    10. In coordination with case management team and/or Clinical Coordinator, oversee client files and records, policy and procedures forms, and submit reports as requested. Assure strictest client confidentiality is maintained.
    11. In coordination with clinical team discuss all client compliance and issues related to client exit (planned and unplanned). Housing Coordinator, whenever possible, will be present to exit a client from the program. When this is not possible, responsibility will defer to Site Director or designee.
    12. Ensure that agency vehicle and transportation guidelines are adhered to per Nation's Finest policy. Transport clients as needed in agency-owned vehicle.
    13. Coordinate activities with the VA Liaison for the program and track progress of any other special grants to ensure compliance with all grant requirements. (Site specific)
    14. Conduct extensive outreach and orientation to other agencies and provider networks to assist in identifying appropriate candidates for Nation's Finest programs.
    15. Respond to program emergency situations on a 24-hour, 7-day basis, and taking alternative time off when required. Mediate resident disputes and consult with the case management team to develop an appropriate disciplinary/corrective action plan when needed.
    16. Other duties as assigned by supervisor.
    Required Job Skills:
    1. Ability to assess eligibility, needs, and barriers facing homeless clients; consult with the clinical team as appropriate to determine placement in an appropriate level of care.
    2. Ability to overcome potential obstacles to any emergency situation that might require immediate response.
    3. Ability to work independently, with minimal supervision while maintain an effective working relationship with other staff members.
    4. Be detail oriented, able to set priorities, meet deadlines, maintain a professional attitude and conduct appearance at all times. Excellent verbal and written communication skills. Ability to convey information in a written format that is accurate in grammar, spelling, and punctuation.
    5. Able to set and maintain professional boundaries at all times with clients adhering to agency policy and procedures.
    6. Excellent inter-personal communication skills with the ability to interact with participants of varying ethnic, social or economic backgrounds. Knowledge of mental health issues, drug and alcohol addiction desired.
    7. Experience working with a homeless population highly desired along with a knowledge of veteran related issues.
    8. Current working knowledge of community resources available to assist clients with achieving goals on their ITP.
    9. Experience conducting case management services.
    10. Proficient in office software including: Word, Excel, Outlook and internet use.
    Qualifications:

    Must have two years' experience in case management within homeless and/or veterans programs.

    Supervisor experience highly desirable.

    AA/AS degree in related field required. Knowledge of local housing and supportive services, and familiarity with local community resources required.

    Applicant must have reliable transportation. Veteran status desirable but not required.

    Offering:

    $ $31.08 per hour depending on funding availability and location, plus full benefits package. Eligible for annual COLA based upon funding availability.
