    Job Summary This position is responsible for administering the digital platform and processes for applicants seeking grant funds from CPCCO.

    Core areas of responsibility include coordination, meeting the funding goals for CCO grant allocations, communication, and technical assistance for key stakeholders.

    This position works closely with the community health partnership director to administer multiple grant programs.
    Essential Responsibilities
    Support clinical and community funding programs, as directed by the CPCCO leadership team.

    Administer and act as lead for multiple CCO grant programs, including sponsorship requests.

    Administer grants program using grants platforms and tracking systems; monitor and manage all application intakes.

    Update and maintain grants materials for both internal and external stakeholders, including community partners.

    Collaborate with internal departments to develop and refine grants materials.

    Review applications for completeness, qualifications and supporting documentation.

    Facilitate materials and meetings for decision and approval process for grant applicants; prepare Proposal Summary for committee recommendations.


    Operationalize decisions, including ensuring Letters of Agreement (LOA), Agreement Summaries and other required documentation are completed and routed appropriately; release funds as specified in the LOA.

    Review required reports as specified in the LOA and follow-up with additional/required information.

    Provide all consultation, Q&A and technical assistance to applicants and reviewers, as needed.

    Communicate with all stakeholders regarding each grant and grant process.


    Work with VP, CPCCO to complete materials required for Board of Directors action and process decisions made by the Board.

    Work with CPCCO leadership to prepare the annual Report to the Community and other grant funding reports.


    Maintain the investment tracker for CPCCO; work with Finance and CPCCO Director to ensure the accuracy of community investment reports, including the Exhibit L report.

    Work with CPCCO Director and key staff to develop relationships and technical assistance with potential applicants.

    Establish and maintain cooperative and productive working relationships with internal and external stakeholders contacted in the course of work.

    Experience and/or Education
    Required

    Minimum 2 years' experience in program or project coordination

    Minimum 1 year experience with grant administration

    Basic knowledge of grant funding processes

    Proficient with Outlook, Word, Excel, and PowerPoint to collect, analyze and display information

    Strong skills with applications and data base platforms for tracking and reporting, such as grant application programs

    Excellent research skills and strong recordkeeping skills

    Excellent listening, verbal, and written composition/expression communication skills

    Critical thinking skills, including the ability to review and administer grants

    Detail orientation and follow-through

    Ability to assess programs and documents to ensure they are fair and equitable
