- Conduct intake screening interviews to assess the need for counseling and care services.
- Assist patients in connecting with community-based services.
- Implement motivational interviewing and trauma-informed approaches.
- Provide case management for patients with mental health and substance abuse disorders.
- Collaborate with community resources and agencies.
- Track patient access and participation in treatment.
- High School diploma/GED with 4 years of behavioral health experience, or Associates degree with 2 years of experience, or Bachelor's Degree with 1 year of experience.
- Current BLS certification and Valid Fingerprint Clearance Card.
- Driver's License and Proof of Insurance may be required.
- Experience as a case manager in substance abuse.
- Ability to effectively communicate in English, bilingual in English and Spanish is a plus.
- Present information clearly to patients and staff.
- Able to move objects up to 10 pounds and operate office equipment.
- Stay in a stationary position and move around as needed.
- Possesses manual dexterity for office tasks.
- Has close visual acuity for computer work.
- Can discern sounds at a normal volume.
- Proficient in math, data analysis, and critical thinking.
- Computer literate and familiar with assessment tools for substance abuse.
- Knowledge of recovery support systems and 12-step programs.
- Understanding of drug and alcohol treatment agencies and resources.
- Shows sensitivity to cultural diversity and workplace collaboration.
- Effective time management and planning skills.
- Health clinic setting with exposure to diseases and chemicals.
- Requires reliable transportation and flexibility with hours.
- Available by phone outside regular hours.
