    Job Title: LPN Case Manager 25-30 hours per week

    Comprehensive Care Delivery

    The Part-Time LPN Case Manager at Joseph's House drives comprehensive care delivery within a multidisciplinary framework.

    • Collaborate with social workers, clinical coordinators, and staff to address the diverse needs of the community.
    • Manage the full lifecycle of member services, including intake, assessment, and complex discharge planning.
