    POSITION PURPOSE

    A qualified Licensed Practical administers skilled nursing care to patients on an intermittent basis in their place of residence. This is performed in accordance with physician orders and plan of care under the direction and supervision of the Registered Nurse. Services are furnished in accordance with Agency policies.

    QUALIFICATIONS, PROFESSIONAL, AND PERSONAL

    1. Graduate of a state approved school of practical nursing and currently to practice professional nursing in Florida.
    2. Minimum of one year experience in nursing, required.
    3. Minimum of one year of home health experience required,
    4. Acceptance of philosophy and goals of this Agency.
    5. Ability to exercise initiative and independent judgment.
    6. Reliable transportation with active automobile insurance throughout employment.
    7. Valid Driver's License with acceptable Motor Vehicle Record upon hire and throughout employment.
    8. Home Care experience
    9. CPR
    MAJOR AREAS OF ACCOUNTABILITY
    1. Understands and adheres to established policies and procedures.
    2. Assists the registered nurse in carrying out the plan of care.
    3. Provides nursing services, treatments and diagnostic and preventive procedures as assigned pursuant to the State Nurse Practice Act
    4. Initiates preventive and rehabilitative nursing procedures as appropriate.
    5. Observes signs and symptoms and reports to the physician and RN reactions to treatments, including drugs and changes in the patient's physical or emotional condition.
    6. Reports any changes in the patient's condition to the registered nurse with the reports documented in the clinical record.
    7. Maintains accurate and complete records of observations, treatments, and care of patient.
    8. Teaches and counsels the patient and family/significant others regarding the nursing care needs and other related problems of the patient at home.
    9. Assists the registered nurse in performing specialized procedures and duties.
    10. Prepares equipment and materials for treatments observing aseptic technique as required.
    11. Assists the patient in learning appropriate self-care techniques.
    12. Participates in orientation, in-services, and staff meetings as scheduled.
    13. Takes on-call duty nights, weekends, and holidays, as assigned.
    WORKING ENVIRONMENT

    Works indoors in Agency office and patient homes and travels to/from patient homes.

    JOB RELATIONSHIP

    Reports directly to the RN case manager/ Clinical Manager.

    PHYSICAL/ENVIRONMENTAL DEMANDS

    Ability to perform the following tasks if necessary:
    • Ability to participate in physical activity.
    • Ability to work for extended period of time while standing and being involved in physical activity.
    • Ability to lift, reposition and transfer patients.
    • Sustain weight of patients using proper transfer safety techniques and equipment (example: use of transfer belt while assisting patient to transfer from bed to chair).
    • Must be able to push, pull, move and/or lift a minimum of 25 pounds to a minimum height of 5 feet and be able to push, pull, move, and/or carry such weight a minimum distance of 50 feet.
