- Under the guidance of the patient's primary care provider, support the mental health care of patients on an assigned patient caseload.
- Screen and assess patients for common pediatric mental health symptoms and disorders. Facilitate patient engagement and determine follow-up care according to acuity.
- Provide patient education about common pediatric mental health disorders and the available treatment options.
- Systematically track treatment response and monitor patients (in person or by telephone) for changes in clinical symptoms and treatment side effects or complications.
- Support psychotropic medication management prescribed by medical providers by monitoring and encouraging adherence and assessing side effects and effectiveness of treatment.
- Provide brief behavioral interventions using evidence-based techniques such as behavioral activation, problem-solving treatment, motivational interviewing, or other treatments as appropriate.
- As appropriate, provide longitudinal talk therapy treatment to patients with higher acuity or more complex diagnoses.
- Facilitate treatment plan changes for patients who are not improving as expected in consultation with the medical provider and the Psychiatric Consultant and who may need more intensive or more specialized mental health care.
- Develop and complete relapse prevention self-management plan with patients who have achieved their treatment goals and are soon to be discharged from the caseload.
- Closely coordinate care with the patients' primary medical provider, psychiatric consultant, and, when appropriate, other mental health providers and applicable individuals.
- Participate in regularly scheduled (usually weekly) caseload consultation with the Psychiatric Consultant and communicate resulting treatment recommendations to the patient's medical provider. Close the loop with the Psychiatric Consultant regarding the status of or changes to the recommended treatment plan.
- Provide or facilitate in-clinic or outside referrals to evidence-based psychosocial treatments (e.g., problem-solving treatment or behavioral activation) as clinically indicated.
- Facilitate referrals for clinically indicated services outside of the organization (e.g., social services such as housing assistance, vocational rehabilitation, mental health specialty care, substance abuse treatment).
- Collaborate with schools to assess symptoms (i.e., Vanderbilt for ADHD) and arrange necessary academic support, learning accommodations for patients as necessary.
- Coordinate with the appropriate practice staff to ensure accurate and timely follow up for patients.
- Leverage an EHR and registry to manage the patient panel and document relevant activities and progress updates.
- Track patient follow up and clinical outcomes using a registry.
- Document in-person and telephone encounters in the registry and use the system to identify and re-engage patients.
- Track time spent on direct patient care, care coordination, and other tasks to enable accurate and timely billing.
- Document patient progress and treatment recommendations in EHR and other required systems to be shared with medical providers, Psychiatric Consultant, and other treating providers.
- Support a culture of quality improvement across practices.
- Listen and incorporate feedback.
- Support and contribute to efforts to improve the overall quality of care across practices.
- Ability to travel to local practices in the surrounding area.
- Ability to work with patients by telephone as well as in person.
- Perform other duties as assigned by their supervisor within this scope.
- Master's degree in Social Work (MSW) from an accredited program required; preference given to candidates holding an active, unrestricted Licensed Clinical Social Worker (LCSW) credential in the State of Georgia
- Minimum of one (1) year of post-graduate professional experience in behavioral health treatment, social services, or care management
- Current and unrestricted licensure as a Licensed Clinical Social Worker (LCSW) in the State of Georgia
- Demonstrated experience in pediatric behavioral health across outpatient, inpatient, or emergency department settings
- Experience conducting comprehensive behavioral health assessments and developing individualized, evidence-based treatment plans for common mental health and/or substance use disorders
- Strong working knowledge of prevalent pediatric mental health and substance use conditions
- Proficiency in evidence-based psychosocial interventions and brief behavioral treatment modalities, including but not limited to motivational interviewing, problem-solving therapy, and behavioral activation
- Foundational knowledge of psychopharmacology related to common pediatric behavioral health conditions
- Experience utilizing digital patient registries or population health management tools to track outcomes and support measurement-based care
- Proficiency with Epic electronic health record (EHR) systems
- Experience working within the Collaborative Care Model (CoCM) or other integrated behavioral health frameworks within primary care settings.

About the Collaborative Care Model:
The Collaborative Care Model is an integrated behavioral health care model designed to treat common mental health conditions in medical settings, like primary care, through a team-based approach. The team is comprised of the patient's Primary Care Provider (PCP), Behavioral Health Care Manager (BHCM), and a Consulting Psychiatrist. The PCP and BHCM collaborate and receive support from a Consulting Psychiatrist to provide evidence-based medication and psychosocial treatment to the patient.
Position Description:
The Behavioral Health Care Manager (BHCM) partners collaboratively with the patient's Primary Care Provider (PCP), Psychiatric Consultant, and the broader interdisciplinary care team to deliver integrated, evidence-based behavioral health services to pediatric patients experiencing conditions such as ADHD, anxiety, and depression. In this role, the BHCM is responsible for coordinating and managing a defined caseload, ensuring seamless integration of mental and physical healthcare services. The BHCM plays a critical role in advancing whole-person care, improving clinical outcomes, and supporting families through proactive care planning, monitoring, and follow-up.
