    *Seeking a part-time Physical Therapist to assist with care in the Okaloosa County, Florida area

    Physical Therapist PT Responsibilities:

    Provide personalized one-to-one care to clients based on individualized Plan of Care

    Monitor and report changes in clients' conditions and needs to the Director of Nursing

    Keep clients' families informed about care plans, health changes, and any concerns

    Physical Therapist PT Requirements:

    Current unrestricted PT license in the state where care is being provided

    Certified BLS CPR(AHA In-Person) and negative TB skin test or chest x-ray

    Minimum of one year documented experience in a home health agency, primary care clinic, or health facility preferred

    Valid driver's license and automobile insurance

    Up-to-date Level 2 Background Check

    Ability to perform physical tasks such as standing, walking, bending, lifting up to 50 pounds, and assisting clients with mobility
