$55,000 to $65,000 Per YearYou'll Be Responsible For:

Overseeing the work of the Social Service DesigneeHelping residents and families adjust and solve problems regarding their social, emotional, and financial needsAssisting with Room and Roommate ChangesAttending Medicare MeetingsDischarge planning for patients by arranging for follow up community services, home health referrals and alternative placements,Advocating for residents and families regarding resident rights issues.

Conducting in-service training to educate staff about the emotional & social needs of residents, and offer methods for better serving residents and familiesAttending administrative staff meetings, morning reports, Performance Improvement meetings, resident care-related meetings, and quarterly staff meetingsMaintaining departmental records and documentation as requiredCoordinating and updating resident care conference scheduleAssisting residents and families with Medicaid approval and reapplicationProviding Medicaid Pending and Patient Liability related follow-upYou'll

Need To Have:
A Bachelors Degree in Social WorkActive Licensed Social Worker Certification in the state where facility is located (OH)
