    This position works in a highly matrix structure to bridge the IU Health provider practices with IU Health Plans, to successfully execute on Value Base Care initiatives. The ideal candidate is self-driven, naturally curious, and have the ability to work within the grey to achieve specific outcomes. Additionally, the ideal candidate is technically savvy to navigate the EMR and have the ability to imply analytical problem solving to improve performance through group and individual training sessions.

    The Intermediate Population Health Consultant coordinates the implementation of Population Health management initiatives across a network of IU Health practices enabling providers to deliver higher quality ambulatory care. Supports IU Health management teams, regional leadership, primary care, subspecialty providers, and practice staff across assigned regions of Indiana to integrate data driven strategies and targeted provider education to provide better health outcomes, lower cost of care, and improve patient and clinician experience. Assists the designing and monitoring of population health initiatives while maintaining strong relationships with providers and staff as well as supports value-based care concepts, initiatives, and strategies.

    • Bachelor's Degree or equivalent years of experience is required.
    • Requires 3-5 years of relevant experience.
    • Proven ability to implement data into practice; respond to analytical insights with strategies and solutions that impact the provision of care, including clinical documentation opportunities; evidence-based decision-making skills.
    • Reliable transportation is critical as travel to multiple locations within a specific region of Indiana is required; travel estimated at 35% - 50% (no overnight travel anticipated).
    • Must have relevant experience working with clinical providers including the ability to leverage data to influence physicians / advanced practice providers and clinical practice behavior by effective delivery of related education to various internal customers.
    • Must have prior experience working in a highly matrixed interprofessional team environment, demonstrating self-driven resourcefulness and excellent time management skills.
    • Requires basic proficiency in Microsoft Office (Word, PowerPoint, Excel, Outlook), and an ability to navigating databases, Microsoft Teams, and conduct basic data analysis.
