Requirements:

• Graduated from an accredited nursing program
• Registered nurse license in the state of practice
• Current CPR and First Aid Certification
• Knowledgeable about quality assessment and assurance procedures
• Valid driver's license and insurance

Working Hours: Monday to Friday, 8:30 am to 5 pm

Location: Fox Lake, Spring Grove

On-Call Schedule: 1 shift on Monday (5 pm to 12 am) and every 10th weekend

Responsibilities:

• Manage patient and family caseload, including assessment, planning, and evaluation
• Gather necessary information related to patients' needs and coordinate with the interdisciplinary team
• Communicate with physicians and agencies to ensure optimal care
• Regularly reassess patient needs and update care plans
• Provide support to families and perform clinical pronouncement of death
• Maintain accurate patient records and prepare care plans
• Supervise care delivery by Home Health Aides
