Department: Health and Human Services
Position Status: Full-time; Appointed; Mn. Teamsters Local 320 Union
Position Location: Chisago County Health & Human Services, North Branch
Position Objective:
The Disease Prevention & Control Program Coordinator is responsible for planning, implementation, and evaluation of processes for the public health disease prevention and control program. This position works as part of a team concerned with disease investigations and immunization services and collaborates with the Community Health Supervisor.
Supervision Received:
Reports to Community Health Supervisor.

Essential Duties and Responsibilities:
Provide overall coordination and management of the public health planning process to implement infectious disease policies and programs and grant deliverables to achieve program objectives and outcomes.
Lead the assessment of the local surveillance and reporting systems to respond to infectious disease outbreaks. Coordinate disease investigation response and work with MDH and other infectious disease partners as required.
Implement disease reporting and prevention protocols as outlined in the Minnesota Infectious Disease Operations Guide and lead the review of Disease Prevention and Control (DPC) policies and procedures such as TB/LTBI, Perinatal Hep B, Refugee Health and other communicable diseases.
Lead training and orientation efforts for Disease Prevention & Control staff, public health staff or Medical Reserve Corps participating in DPC related activities. Take lead in DPC outreach, education and disease reporting with medical providers, schools, business, and others as identified. Outreach and promotion activities to include Community Events (Health Fairs, County Fairs, School Open House, Work site wellness events, etc.) Coordinate and train staff on various electronic systems such as HAN, PH-DOC and databases including MEDSS and MIIC.
Coordinate Vaccine Programs; Manage MnVFC and UUAV vaccine programs; Reviews and maintains vaccine management plans, protocols and policies, including physician orders. Manage immunization clinics including scheduling and training of staff and overseeing vaccination grant programs. Role as administrator for Minnesota Immunization Information Connection (MIIC)for Chisago County. Includes data entry, maintenance of inventory, reports. clinic/school notify of immunizations done to healthcare providers, schools and parents. Provides training, as needed, for school staff on MIIC and assists with access issues.
Coordinate and support work between Disease Prevention & Control and Emergency Preparedness work units to increase response capacity as work demands fluctuate. Assist in developing of Emergency plans such as isolation and quarantine, mass dispensing etc. Assists in Emergency Preparedness Training plans. Serve as Operation Chief for Medical Countermeasures, Designated 24/7 Health Alert Contact.
Provide consultation to local, regional, and statewide partners and serve as a resource for disease prevention and control education and updates. Collaborate with partners to increase immunization rates.
Collect and analyze program data, prepare local, regional, and statewide program reports, plans and grant applications that support grant deliverables, program objectives and overall department needs.
Coordinate and train staff on various electronic systems such as HAN, PH-DOC and databases including MEDSS and MIIC.
Assist with coordination of Public Health student clinical rotations and/or internships.

Education and Experience:
Registered Nurse, BSN prepared with PHN certificate preferred. 2 years of paid BSN nursing experience. RNs with 2-year degree require 3 years of paid RN nursing experience.
OR
Bachelor's degree in natural sciences or related degree, with 2 years' experience in infection prevention and control, communicable disease work or epidemiological investigations and control.
OR
Master's degree in public health, Epidemiology or related degree with 1-year paid work experience.
Tools and Equipment Used:
Blood pressure cuff, syringes, desk computer, laptop, telephone, cell phone, copier, scanner, fax machine.

Work Environment:
The work environment is a combination of both the office and the field. The noise level can range from moderately quiet in the office to loud and noisy in the field.
The employee occasionally may be exposed to contagious diseases and parasites and/or potentially violent individuals or domestic animals.
