Job Title: Registered Nurse - Home Care

Location: Phoenixville, PA

Duration: 13 Weeks

Shift Timings: 8 AM - 4:30 PM

Job Description:

• Conduct initial patient assessments to develop individualized care plans tailored to the patient's needs and preferences.


• Provide skilled nursing care, including wound care, medication management, and symptom management, in the comfort of patients' homes.


• Educate patients and their families on disease management, medication administration, and other aspects of care to promote independence and improve outcomes.


• Collaborate with other healthcare professionals, including physicians, therapists, and social workers, to coordinate comprehensive care for patients in the home setting.


• Monitor patients' health status and response to treatment, adjusting care plans as needed to optimize outcomes and ensure patient safety.


• Document all care provided accurately and timely, maintaining thorough and up-to-date patient records in compliance with regulatory standards and agency policies.

• PMAH is looking for a Registered Nurse with a minimum of 2+ years of experience.


• Candidate should have homecare experience that includes doing 4-6 revisits per day.


• Candidates with Private Duty or Assisted Living experience will not be a fit for this role.


• Nurse must have Oasis experience with documentation.


• The following are the coverage areas we are looking for: Northeast Philly and Montgomery County


• Note – if this is a traveler it must be indicated in the profile. Candidates will be declined for living more than 30 minutes from the territory/zip code.
