    We are currently seeking a compassionate and dedicated Registered Nurse (RN) or Licensed Practical Nurse (LPN) to join our team in the role of Hospice Clinical Liaison in Denver, Colorado.

    The Hospice Clinical Liaison plays a crucial role in our organization as the main point of contact for processing patient referrals from intake to admission. In this position, you will build and maintain positive relationships with patients, families, and healthcare professionals to ensure a seamless assessment process for hospice eligibility. Working closely with our Account Executives, you will assess patients, provide education on hospice services, and establish networking connections with various healthcare providers in the community.

    Responsibilities:

    • Develop and maintain relationships with healthcare facilities to promote awareness of hospice services
    • Evaluate patients for admission and provide clinical support
    • Verify insurance coverage and collaborate with healthcare teams
    • Act as a liaison between hospitals and hospice teams
    • Participate in quality improvement programs and staff training
    • Prepare reports and ensure compliance with regulations
    • Respond to emergencies and ensure patient safety

    Qualifications:

    • Bachelor's degree preferred; Associate's degree in nursing or LPN certification required
    • Valid RN or LPN license in Colorado
    • Strong communication skills and healthcare experience
    • Computer proficiency and reliable transportation
    • Knowledge of Medicare managed care and insurance reimbursement

    Salary for LPNs ranges from $31 to $36 per hour, while RNs can expect a salary between $74,000 and $83,000 annually.
