- Experience working in a long-term care environment.
- Proficiency in Electronic Medical Records (EMR).
- Ability to manage patients with advanced illnesses and multiple chronic conditions.
- Capability to collaborate within an interdisciplinary care team for coordinated patient care.
- Conducting assessments of patients and their medical histories upon admission to the nursing home.
- Developing comprehensive care plans.
- Managing transitional care for patients transitioning from hospitals.
- Offering primary and preventive care for long-term care residents in skilled nursing facilities.
- Supervising residents with sudden changes in condition.
- Providing advanced care planning and chronic care management.
- Prescribing appropriate medications, including controlled substances, following applicable regulations.
- Ordering, reviewing, and monitoring treatments and diagnostic tests.
- Maintaining regular communication with nursing home staff to discuss changes in condition and care plans, as needed.
