Summary

The Registered Nurse delivers fundamental, knowledge-based care to assigned patients and effectively applies all aspects of the nursing process with collaborative, interdisciplinary, evidence-based practice which will enable effective care coordination and implementation of health promotion and disease prevention practices.

Duties

Major Duties Include:

Participation in Huddles and use of the Scrubbing Schedule-Clinic Huddle Planning Tool
Complete Pre-operation procedures, including coordinating with scheduling clerk, entering orders, following to completion
Handles Alerts, Discharge Calls, & Secure Messages when covering for another Registered Nurse(RN)
Vista/Fax Review
Determining if discharge follow ups can be done by RN instead of provider
Flagging provider notes and documenting all patient conversations
Secure Messages
Discharge Follow Up Calls
Face-to-face and Phone Clinics-close all encounters
Complete Clinical Reminders at every visit
Entering notes when flagging medication
PACT No Show Telephone Note for all face-to-face (F2F)no shows. Notifies Medical Support Assistant so appointment can be taken off schedule to decrease number of open encounters
PDMPs, UDS/Med Match, Informed Consent, Naloxone
Setting Surrogate when on Leave
Community Care Coordination Plan Note-if notified by patient of NVCC hospital visit
Return to Clinic Orders and order any necessary labs needed for next visit
Coordination of patient health care needs
Providing information to patients: to include clinic policy on walk-ins, education to patient/caregiver related to medication, diagnostic studies and results, and disease process and prevention
Follow protocols for entry of orders and/or consults
Comply with care team functions
Complete patient documentation in the Computerized Patient Record System (CPRS)
Determine the necessity for immediate medical attention and contact the appropriate PCP
Make referrals to appropriate ancillary services as per protocol for patient assistance in accordance with consult template service line agreements
Use established diagnoses for RN Care Management visits to complete encounter forms
Scrub provider list 48 hours in advance
Review and prepare incoming medical records for providers
Assist and oversee LPN
All other duties as assigned, pertaining to this occupation, to meet the needs of the Agency, Department, Facility, and/or Service
