Transition Team Nurse - Pittsburgh, United States - Maxim Healthcare Services
Description
Transition Team nurses provide care for all authorized hours in a home during a provisional period between 1-120 days. During this time, our office staff will be recruiting to identify full-time, replacement home healthcare nurses. The new caregivers will have an opportunity to train and shadow in the home with the transition team caregivers prior to working independently with the patient. Once the replacement nurses are trained and have a schedule, the Transitions Team are then phased out of the home and onto the next. Not just anyone can be a transition team nurse. You are the face of the company. You are the first person the family thinks of when they are new to homecare. You make a difference. This is a rare opportunity
Transition Team Function
Skilled nurses designated by region that are utilized on referrals only with the primary purpose being transitioning patients from hospital to home.
To be deployed on primary referral only
-Children's Hospital
-Children's Home
-Health plan partnerships
Health Plans/Single Case Agreements on occasion
Expectation is that the team is fully replaced/removed from the home after 90 days of service and a 30 day grace period
Transition Team members are signed to a local region.
Based on team member need and location, some TT may support nearby regions when applicable.
Transition Team Requirements
Minimum one year of nursing experience
Must be approved for TT by internal RN during office interview
Must pass office competency/skills validation on the following items:
*Trach
*Vent
*G-Tube
Proper Documentation
Ideal candidate will display above average customer service, communication, and professionalism qualities.
Candidate must display understanding of Maxim's emphasis/requirements of the following items:
Consistent communication with internal staff
Ongoing vigilance with securing PHI, meeting all HIPPA regulations, and security of patient and staff privacy.
Ability to maintain a schedule that will change frequently
Possibility of lengthy commute times to and from work
Fortitude to work in varying home environments
Must be able to work a minimum of 4 weekend shifts a month
Transition Team Pay Scale
*No Novice Nurse candidates will be accepted for the Transition Team, no exceptions*
