- Know and follow all procedures and policies of Waterford Place Assisted Living; enforce these policies and procedures with all staff.
- Ensure that only trained and authorized personnel operate Waterford Place equipment.
- Initiate and facilitate resident care planning process and ensure timely updates as determined necessary by the interdisciplinary team.
- Support admissions and orient new residents; follow complete admission procedures.
- Coordination of required appointments.
- Coordination of service plan resident reviews (biannual schedule) and facilitation of service plan review meetings with residents and resident representatives.
- Participation COPC weekly meetings and spreadsheet collaboration.
- Resident satisfaction, clinical process audits-QAPI participation.
- Completion and compliance of Health & Service Evaluation (HSE).
- Facilitation of H&P spreadsheet for timeliness and regulatory compliance.
- Completion and compliance of Self-Administration Compliance Assessment.
- Regular audit (daily/weekly) shower sheets/stop & watch tool against shower schedule.
- Coordination of resident supply needs with central supply and clinical staff including accurate assignment of charge codes using scanners.
- Collaboration between billing office and central supply for reconciliation of resident medical supplies purchased by facility.
- Supply room organization and par level management.
- Facilitation of corrective action process (when necessary) and conflict management settings.
- Facilitate performance evaluations and staff meetings.
- Assist with recruiting and hiring practices in collaboration with Workforce Coordinator.
- Orient new clinical staff members, facilitate completion of orientation checklist.
- Participate in monthly department meetings.
- Collaborate with care partnerships (hospice, home care services, resident associated guardians/POAs etc.)
- Primary educator for Waterford Place Assisted Living staff, residents, guests.
- Understand and facilitate compliance for residential care facility regulations in accordance with the Ohio Department of Health.
- Maintain survey readiness facility book.
- Ensure survey review book is available and up to date with past 3 cycles of surveys.
- LNHA or LPN currently licensed in the State
- At least 2 years of experience required, long term care setting preferred
- At least 1 year of prior management experience
- Excellent verbal, written, and interpersonal communication skills
