    Vetted is seeking a Social Worker for a travel job in Beaverton, Oregon.

    Must have 2+ years of experience.

    This contract pays approximately $12,154/month gross.

    Assignment details:

    • Contract length: 16 weeks
    • Shift length: 8 hour shifts
    • Hours per week: 40
    • Start Date: ASAP

    Job responsibilities:

    • Communicate changes in discharge needs to the care team promptly.
    • Conduct psychosocial assessments and identify barriers impacting care transitions.
    • Screen for social determinants of health and incorporate findings into care plans as applicable.
    • Maintain confidentiality and follow established documentation standards.
    • Work 40 hours per week with 8-hour shifts.
