    We are seeking a dedicated and compassionate Targeted Case Manager to join our team at Horizon Health Services.

    Your Day-to-Day:

    • Support patients through outreach calls, telehealth, and face-to-face visits to identify barriers and connect them to community resources and services.

    • Provide transitional care, including follow-up outreach to link patients from inpatient, emergency room, or residential treatment settings to other providers and services.

    • Be an advocate for comprehensive care and connect patients with necessary services.

    • Assist patients in navigating complex healthcare and social service systems by providing support services and linking them to relevant resources.

    • Retrieve clinical and non-clinical data to help patients achieve their goals using internal Electronic Health Record (EHR) systems and regional health information systems.

    • Develop strong relationships with patients from diverse backgrounds, families, and communities, and interact with them in a positive, supportive, and cooperative manner.

    • Stay up-to-date on Horizon Health Services Policy and Procedures, and adhere to confidentiality and privacy regulations.

    Location: Our clinic is located at 3020 Bailey Ave in Buffalo, NY, and frequent local travel is required.

    Hours: Monday through Friday, day shifts, with some flexibility to work evenings as needed.

    Compensation: $20.00 - $24.00 per hour, depending on experience and education.
