Job Summary
We are seeking an experienced Family Nurse Practitioner to join our thriving behavioral health team. This role will primarily support the Greater Lakes Homeless Outreach Stabilization and Transition (HOST) program delivering field-based care to adults facing serious behavioral health challenges.Responsibilities
- Provides psychiatric services to clients enrolled in community outreach programs including medication administration and monitoring as well as general nursing care and patient education. ,
- Collaborates with internal and external medication prescribers and other clinical staff to ensure effective patient care. ,
- Maintains a flexible schedule during scheduled shift depending on patient needs.
