    Description
    Physical Therapist needed for home health setting surrounding Newton, MAAbility to travel to cases as assigned.

    Provide physical therapy services to patients in their place of residence who have physical impairments, functional limitations or disabilities resulting from injury, disease, or other causes.

    Collaborate with patient, family, physician, and other allied health professionals to identify patient needs and coordinate services to meet those needs.

    Utilizing thorough and timely electronic documentation and interdisciplinary communications in accordance with agency policies, procedures, and standards of practice.


    Physical Therapy Licenses/Certifications:
    CPR/BLS (Basic Life Support), State License-Physical Therapist-Massachusetts, NPI NUMBER

    Physical Abilities:
