    What will you do?
    As the

    Service Coordination Manager


    and future leader in our organization, you will focus on driving communication and increasing the quality of homecare service to clients while facilitating a constructive and encouraging environment for field clinician team members.

    You will manage a casebook of patients, where you'll build out their schedules by coordinating between the families we serve and the availability of our field clinician team member.

    Our core values of telling the truth, owning the details, and raising the bar will require the Service Coordination Manager to focus on building relationships with clients and community partners to fortify the culture Family First Homecare values as an organization.


    Essential Duties and Responsibilities:
    Maintains a current patient roster or "casebook" with all pertinent staffing information and monitors schedule changes.
    Performs telephone screens and face to face interviews with potential field clinician team members.
    Prepares monthly schedule for clients and field staff and provides copies for distribution.
    Assists in coordinating services provided to patients.
    Controls and monitors schedule changes.

    Checks compliance of shifts and visits done (on master schedule) after all notes are matched to charges, brings any scheduling problems to the supervisor's attention immediately.

    Assists in taking patient referrals.
    Manages established key performance indicators to drive operational efficiencies and growth within the business unit
    BA or BS preferred.
    Preferably possesses healthcare experience.
    Should be a skilled organizer able to manage office files, logbooks, and staff schedules.
    Must be able to execute high volume of phone calls and utilize appropriate phone etiquette.
    Establish and maintain effective channels of communication with patients and field clinician team members.
