- Responsible for carrying a caseload of 25 clients
- Develops therapeutic relationships and utilizes concepts of reflection and motivational interviewing with women and their families in a home visiting environment.
- Performs home visits in accordance with the NFP model, visit-to-visit guidelines, and agency procedures.
- Follows the nursing process in implementing NFP into nursing practice. Formulates nursing diagnosis based on nursing assessment and client goals.
- Evaluates client progress and plan home visits in accordance with stated goals and NFP outcomes.
- Assesses physical, emotional, social, and environmental needs of women and their families as they relate to the NFP domains.
- Provides education, support, and referral resources in assisting clients and their families to attain their targeted goals.
- Consults and collaborates with other professionals providing services to clients and their families.
- Meets with NFP Nurse Supervisor weekly for clinical supervision.
- Conduct outreach and recruitment activities for the program and the department throughout the year.

- Two years' recent experience in maternal/child health, public health, home visiting or mental/behavioral nursing.

Essential Functions
Nurse-Family Partnership empowers first-time moms to transform their lives and create better futures for themselves and their babies. Nurses in this role are compassionate, organized, flexible, and self-directed, with strong advocacy and teaching skills. This role provides home visiting services to eligible first-time mothers and their families.

Generally, the normal work hours for the university are Monday through Thursday, 7:30 - 5:30 pm; Friday, 8:00 am - 12:00 pm with a one-hour lunch.
The individual holding this position may need to be available early morning, evening, and on weekends to meet the needs of the department.
